Join Our Team: Community Case Manager – Hillsboro, KS

Prairie View is looking for a Community Case Manager to provide meaningful, hands-on support in schools, homes, and the community. What You’ll Do:

Deliver community-based services to children and adolescents and their families, guided by individualized treatment plans.

Highlight patient strengths, build self-esteem, and help families achieve their goals.

Provide crisis intervention, teach life and parenting skills, and connect families with community resources.

Attend team meetings, collaborate with colleagues, and communicate regularly with referral sources.

Participate in Prairie View’s Summer Intensive Rehabilitation Program (SIRP) as needed.

Educate the broader community about children’s behavioral health services.
What We’re Looking For:

Ability to work effectively with children, adolescents, and their caregivers, understanding typical vs. atypical behaviors.

Strong problem-solving, crisis intervention, and advocacy skills.

Flexibility to meet families’ needs where and when they need support.

Valid Kansas driver’s license, current auto insurance, and ability to transport patients safely.

Administrative Responsibilities:

Maintain timely and accurate documentation of all patient interactions.

Participate in department meetings and clinical supervision.

Qualifications:POSITION QUALIFICATIONS:
- Minimum Education: Bachelor’s degree in related field of behavioral health sciences or equivalent
- Minimum Experience: Demonstrated skills in the provision of case management
- Must be at least 20 years of age
- Must have good listening skills, verbal skills, and written communication skills
- Must be able to observe patient demeanor and nonverbal communication for assessment purposes
- Must be able and willing to have some flexibility in scheduling, to meet patient and organization needs
- Must maintain Kansas State licensure and appropriate CEU’s where applicable
- Must be able to communicate effectively with referral sources and external care providers
- Must have ability to work effectively with children, adolescents, and their families
- Must be able to understand the special needs and behaviors of and be able to care for children and adolescents ages 2 to 18 (up to 22 years old if on the Waiver)
- Must have a valid Kansas driver’s license and be insurable through Prairie View insurance company.
- Must be able to pass physical restraining program
- Must be able to bend, squat, and kneel to do physical activities with children
- Must be proficient at basic keyboarding/word processing skills
- Must be able to pass Kansas Bureau of Investigation Screen, Central Registry Screen, and DCF (Kansas Dept. for Children & Families, formerly SRS) background check
