The Heart of the Role
As a Care Coordinator, you are the bridge between clinical expertise and real-world success. You will work closely with adults participating in community support services, helping them navigate their mental health journey with dignity and autonomy.

Relationship Builder: Develop deep, supportive bonds with clients, becoming a consistent presence in their recovery.

Empowerment Advocate: Help clients identify their unique strengths, increase their community involvement, and learn effective ways to manage emotional challenges.

Strategic Planner: Facilitate treatment plan meetings every 90 days, ensuring goals remain relevant and that clinical oversight from therapists is seamlessly integrated.

Community Liaison: Build a "natural network" for your clients by fostering relationships with local agencies and services.

Safety & Support: Provide vital crisis intervention and maintain a safe, boundary-respecting environment for both individual and group work.
What You’ll Do (The Essentials)

Clinical Excellence: Assess client needs, update treatment plans, and provide clear interpretations of service choices.

Collaborative Care: Work hand-in-hand with therapists, case managers, and attendant care workers to ensure a holistic approach to health.

Administrative Precision: Maintain timely documentation (90% of notes within 72 hours) and coordinate with our Utilization Review team to ensure services are always authorized and active.

Growth-Oriented: Participate in professional development, including State of Kansas Case Manager training and ongoing workshops to sharpen your skills.

Qualifications:POSITION QUALIFICATIONS:
- Minimum Education: High school diploma and Bachelor’s degree or other approved work/personal experience in working with adults
- Minimum Experience: Two years working with adults who have severe and persistent mental illness
- Must have good listening skills, verbal skills, and written communication skills
- Must be able to observe patient demeanor and non verbal communication for assessment purposes
- Must be at least 21 years of age
- Must be able and willing to work flexible hours and adjust work schedule to meet patient and organization needs
- Must hold and maintain State of Kansas Case Manager Certification and appropriate CEU’s where applicable
- Must be able to communicate effectively with referral sources, external care providers, and mental health center team members (i.e. case managers, therapists, physicians, community support workers, teachers, pastors, coaches)
- Must have ability to work effectively with adults with mental illness
- Must have ability to work effectively with adults who have severe and persistent mental illness and provide support
- Must be proficient at basic keyboarding/word processing skills
- Must be able to pass Kansas Bureau of Investigation Screen, Central Registry Screen, and DCF (Kansas Dept. for Children & Families, formerly SRS) background check
