MDS Coordinator Job Description
Job Title: MDS Coordinator
Department: Nursing
Reports to: Administrator
SUMMARY: The primary purpose of the position is to ensure the highest quality of resident care available, support staff and establish a positive reputation in the community. Ensure systems and processes are effectively implemented which promote coordination of care, patient advocacy and accurate reimbursement for services rendered.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Adherence to Policies
· Must show courtesy and respect to other employees, residents, family members, visitors, government agencies, the general public, etc.
· Abide by Eduro HealthCare’s drug and alcohol policy.
· Maintain good personal hygiene and dress and groom appropriately.
· Report on time as scheduled and follow all company policies and procedures.
· Attend staff meetings and in-service sessions.
· Become thoroughly familiar with emergency procedures.
· Must be able to perform duties in a timely fashion, and within the prescribed sequences and schedules.
· Must be cooperative with other departments and be courteous and respectful in dealing with them at all times.
· Participate in and respond professionally to surveys (inspections) conducted by government agencies.
Administrative Duties include
· Assume the authority, responsibility, and accountability necessary to carry out the duties of this department.
· Take responsibility for the entire facility using knowledge of long-term care.
· Show leadership skills and the ability to lead an motivate others.
· Supervise and coordinate all departments, supporting and enabling each one to function at the highest standard of performance.
· Act as a liaison between departments and shifts to ensure cooperation, efficiency, mutual respect and teamwork.
· Complete paperwork requirements on schedule and in an accurate, appropriate manner with attention to detail.
· Maintain good public relations and good interdepartmental relations.
· Demonstrate loyalty to the health care profession, the facility and to the management.
· Focus the combined expertise and effort of all the committees, consultants, physicians, other professionals and staff to ensure maximum quality of environment, medical treatment, comfort, dignity and life for each resident.
Skilled Service Coordination – Delivery of Care Duties include
· Supervises facility Utilization Case Mix Manager initiation of resident plans of care which accurately reflect the skilled services rendered and that such services meet the coverage criteria for the identified RUG/reimbursement category.
· Promotes facility Utilization Case Mix Manager efforts to coordinate resident care through oversight of compliance with resident care plan by responsible facility personnel. Ensures adherence to company policies and procedures pertaining to documentation requirements to support the care and services provided.
· Ensures facility Utilization Case Mix Manager participation at the weekly and monthly case management committee meetings and appropriate follow-up of recommendations derived from committee meetings.
· Initiates resident plans of care which accurately reflect the skilled services rendered and ensures that such services meet the coverage criteria for the identified RUG/reimbursement category.
· Coordinates resident care through oversight of compliance with resident care plan by responsible facility personnel.
· Ensures adherence to company policies and procedures pertaining to documentation requirements to support the care and services provided.
· Chairs weekly and monthly case management committee meetings and ensures appropriate follow-up of recommendations derived from committee meetings.
· Complies with company policies and procedures for weekly/monthly triple check by facility interdisciplinary team for the verification of accurate utilization data and support documentation for the services to be billed.
Facility Support Duties include
· Serves as a resource for facility Utilization Case Mix Managers pertaining to effective Medicare/Managed Care case management, Medicaid Case Mix and other reimbursement issues as required.
· Provides ongoing training and education to facility personnel as required. Reviews the status of all Medicare/Managed Care residents with the facility Utilization Case Mix Manager weekly.
· Provides consultation and guidance to facility managers to determine the appropriate discharge of Medicare/Managed care residents from services.
· Acts as the Utilization Case Mix Manager in the absence of such.
· Assists facility managers to respond appropriately to requests for information by the state, fiscal intermediary or managed care provider.
· Ensures that appropriate information is gathered and submitted in a professional and timely manner.
· Serves as a facility resource for interpretation of federal, state and local regulations and to implement systems that ensure regulatory compliance.
· Provides onsite support to the facility during survey visits conducted by regulatory agencies.
· Assists facility management with the development of applicable plans of correction which addresses deficiencies cited.
· As appropriate, acts as an intermediary between the facility, physicians, consultants and ancillary services.
Patient Care
· Be personally involved with all aspects of resident care to the extent necessary to assess its quality.
· Respect all resident rights, including the confidentiality of resident care information.
· Report immediately to the proper legal authorities if you have reason to believe a resident has been physically, emotionally, or sexually abused, or been a victim of theft of their personal property.
· Create and maintain an atmosphere of warmth, cheerfulness, enthusiasm, and love, giving the resident the quality of service you would want to receive personally.
QUALIFICATIONS

Education and Experience
· Experience working with adults in a health care setting may be required
· At least an Associates in Nursing required
Language Skills
· Must be able to read, write, speak, and understand English sufficiently to perform required duties.
· Must be able to follow verbal and written instructions in English.
· Bi-lingual skills welcome
Certificates, Licenses, Registrations
· Must be a Registered Nurse or Licensed Practical (Vocational) Nurse in good standing in state employed
· Successful completion of a training and competency evaluation program required
· Completion of orientation required by company policy.
· Health screening and TB test (if required) within two weeks of employment.
· Mandatory drug test within 90 days of hire.
· Must be free of criminal activity proven by a criminal background check.
· Become familiar with and comply to all local, state, and federal regulations relating to the job.
· Show within three (3) days of hire satisfactory evidence of identity and eligibility for employment.
· Ongoing pursuit of Continuing Education Credits in clinical subjects, management, personal growth and development.
Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. The employee must be able to perform the essential job functions without posing a direct threat to residents, self or others.
· Frequently move about the facility, transporting equipment and supplies upwards of 50 pounds.
· Frequently assist patients with exercises and move them about the facility and between bed, wheelchair, and other positions as necessary.
· Must be able to rapidly assist in the evacuation of all residents from the building in case of emergency.
Work Environment
· May be asked to work beyond normal working hours and must have flexibility and willingness to work holidays, etc.
· Must work in an office and other areas of the facility as needed.
· Is subject to illness, diseases, and other health conditions present in the facility.
· Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
