Job Overview
We are seeking a dedicated MDS Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring compliance with regulatory requirements and enhancing the quality of patient care. This position requires a strong understanding of clinical documentation, coding, and care management processes, as well as experience in various healthcare settings.
Duties
- Conduct comprehensive assessments using the Minimum Data Set (MDS) to evaluate patient needs and care plans.
- Collaborate with interdisciplinary teams to ensure accurate documentation and compliance with Medicare and managed care regulations.
- Review medical records for accuracy and completeness, ensuring adherence to HIPAA standards.
- Utilize EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for documentation and patient management.
- Engage in utilization review processes to optimize patient outcomes and resource allocation.
- Provide education and training on clinical documentation improvement and coding practices (CPT, ICD-9, ICD-10).
- Participate in discharge planning and case management activities to facilitate smooth transitions of care.
- Maintain knowledge of NCQA standards and best practices in health information management.
Experience
- Previous experience in critical care, acute care, or home care settings is preferred.
- Familiarity with medical terminology, anatomy, physiology, and medical records management is essential.
- Experience with nursing home operations or level I/II trauma center environments is a plus.
- Background in hospice care, outpatient services, or emergency medicine will be considered advantageous.
- Strong skills in documentation review, medical coding (CPT/ICD), and clinical data analysis are required.
