Post Rehab is hiring a Speech-Language Pathologist (SLP) to join our skilled nursing rehabilitation team. This role offers the opportunity to work closely with residents to support communication, cognition, and swallowing while making a meaningful impact on daily function and quality of life.
Position Responsibilities:
- Evaluate residents for speech, language, cognitive, and swallowing needs
- Develop individualized treatment plans based on clinical findings and resident goals
- Provide therapy focused on communication skills, cognition, and dysphagia management
- Collaborate with nursing, rehab staff, physicians, and families regarding resident progress and care planning
- Complete timely and accurate documentation in accordance with facility standards
