We are seeking a Psychiatric Nurse Practitioner (PMHNP) or experienced Behavioral Health Nurse Practitioner to join our growing team serving patients in skilled nursing facilities and senior living communities.

Position Summary
The Psychiatric Nurse Practitioner (PMHNP) provides psychiatric evaluation, medication management, and behavioral health care to patients in skilled nursing and senior living settings. The provider works closely with facility leadership, interdisciplinary teams, and primary care providers to coordinate comprehensive patient care.
Responsibilities
- Conduct psychiatric evaluations and follow-up visits
- Diagnose and treat behavioral health conditions including depression, anxiety, and dementia-related behaviors
- Provide medication management and treatment planning
- Accept patient referrals from facility staff and medical providers
- Coordinate care with attending physicians, primary care providers, and interdisciplinary teams
- Participate in care planning and discharge planning
- Monitor patient progress and adjust treatment plans accordingly
- Order and interpret diagnostic testing when appropriate (labs, imaging, EKGs)
- Provide education to patients, families, and nursing staff regarding behavioral health conditions
- Comply with all applicable federal, state, and regulatory standards
Qualifications
Required
- Licensed Nurse Practitioner
- Active state license and DEA (or ability to obtain)
- Minimum 2 years psychiatric or behavioral health experience
Preferred
- PMHNP certification (Psychiatric Mental Health Nurse Practitioner)
- Experience in skilled nursing, geriatrics, post-acute care, or senior living
- Experience treating geriatric psychiatric conditions
