What are the responsibilities and job description for the Youth Services Case Manager position at Positive Growth Inc.?
Youth Services Case Managers: This is a contract position, which involves working with children and youth providing skill building as well as coordination and linkage to outside supports such as school, medical, and psychiatric supports, etc. The Youth Services Case Managers must be able to work during the school day and willing to meet with clients at school. Completion of a bachelor’s degree from an accredited college or university in social work, psychology, counseling, criminal justice is preferred. Direct experience can substitute for education on 1:1 year ratio (Bachelor's = 4 years of relevant experience). This position requires in-person contact. Minimum caseload for part-time is 15 clients. Fulltime is 20 to 35 clients.
Responsibilities:
- Provide support for youth with mental health conditions and connect them with the services and resources needed to meet their treatment goals.
- Assumes the responsibility of assessing needs, identifying adequate resources, discussing these resources with client and making the connection in order to meet the client’s needs
- Conducts needs assessments and goal creation with clients
- providing assistance to by advocating, teaching, demonstrating, monitoring, and/or role modeling new, more functional skills
- community linkage to various resources, education, advocacy
- coordinating with internal and external entities such as therapists, doctors, schools, and the legal system
- discharging and transition planning
- participating in specific treatment meetings and staffing
- accompanying client to specific community-based appointments
- complete required daily documentation
- attending weekly treatment team meetings
