Job Description: ECM Case Manager
Organization: PATH Poor And The Homeless – Navigation Center
Location: Red Bluff, California
Program: CalAIM Enhanced Care Management (ECM)
Reports To: Program Manager / Director of Operations
Pay Starts: $25.00
Position Summary
The ECM Case Manager provides intensive, community-based care coordination services to individuals experiencing homelessness and complex health and social needs. Under the CalAIM initiative through Partnership HealthPlan of California, the ECM Case Manager supports clients in accessing medical, behavioral health, housing, and social services to improve overall stability and health outcomes.
This position requires field-based engagement, strong advocacy, and the ability to work with high-acuity populations in a trauma-informed and culturally responsive manner.
Key Responsibilities
Care Coordination & Case Management
- Provide comprehensive, personcentered case management services to ECMenrolled clients
- Conduct assessments, develop individualized care plans, and track progress
- Coordinate services across physical health, behavioral health, substance use, and social services
- Assist clients in navigating MediCal and CalAIM services
Client Engagement & Outreach
- Conduct outreach in shelters, encampments, and community settings
- Build rapport and maintain consistent engagement with highacuity clients
- Maintain required frequency of contact per ECM guidelines
Housing & Stability Support
- Assist clients in securing and maintaining stable housing
- Support housing applications, documentation, and landlord coordination
- Collaborate closely with PATH Navigation Center staff and local housing providers
Documentation & Compliance
- Complete timely, accurate documentation in compliance with CalAIM and
Partnership HealthPlan of California requirements
- Maintain case files, service logs, and outcome tracking
- Ensure all services are properly documented for program compliance and audits
Interdisciplinary Collaboration
- Participate in case conferences and team meetings
- Coordinate with healthcare providers, behavioral health professionals, and community partners
- Utilize interpreter services to ensure equitable access for all clients
Advocacy & Support
- Advocate for client access to healthcare, benefits, and supportive services
- Assist with scheduling appointments, transportation, and followup care
- Provide crisis intervention and deescalation support when needed
Knowledge
- Knowledge of completing and submitting TAR’s
- Familiarity with PointClickCare, WS_FTP Web Transfer and PHC Online
Services
- Accucare billing program or similar billing programs
Minimum Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field (or equivalent experience)
- 12 years of experience working with homeless or highrisk populations
- Valid California driver’s license
Preferred Qualifications
- Experience with CalAIM or Enhanced Care Management (ECM) programs
- Familiarity with Tehama County resources
- Experience with HMIS or electronic documentation systems
