Case Manager – Behavioral Health Bridge Housing (BHBH) / Transitional Living Programs

Salary

$22.00 – $25.00 per hour, dependent on experience

550 Lay Ave, Red Bluff, CA, and off site bridge housing and transitional housing locations

Position Summary (BHBH Aligned)

The Case Manager provides housing focused, voluntary, trauma informed case management services to individuals and families experiencing homelessness, including individuals with mental illness, substance use disorders, and co-occurring disorders in alignment with the DHCS Behavioral Health Bridge Housing (BHBH) Program

Under Assembly Bill 179:

Services are delivered using Housing First, harm reduction, and client centered practices, with the primary goal of stabilization, engagement in behavioral health services, and transition to permanent or long-term housing. The Case Manager screens applicants for placement, conducts ongoing needs assessments, coordinates supportive services, and removes barriers to housing stability while ensuring program compliance and participant safety.

The Case Manager is responsible for:

  • Supporting participant stabilization
  • Assist clients in removing barriers to successful transition into permanent sustainable housing
  • Coordinating medical and behavioral health services
  • Coordinating housing services
  • Addressing safety or compliance concerns
  • Initiating removal of individuals who pose a threat to others or program operations, consistent with agency policy and participant agreements
  • Ensuring clients have the skillset necessary to obtain and maintain permanent sustainable housing

Essential Duties and Responsibilities

Client Engagement, Intake & Assessment

  • Screen, assess, and determine eligibility for placement in Transitional Living and BHBH funded programs.
  • Conduct initial and ongoing comprehensive assessments addressing housing status, behavioral health, substance use, income, benefits, and life skills.
  • Provide clear orientation to program expectations, rights, and voluntary services.

Case Management & Service Planning

  • Manage an assigned caseload of Program Participants.
  • Develop, implement, and update Individualized Housing and Service Plans with measurable goals focused on stability, recovery, and housing exits.
  • Meet individually with each Program Participant for a minimum of thirty (30) minutes weekly to review progress and revise plans.
  • Provide advocacy to support income growth through benefits enrollment, employment referrals, and community resources.

Housing Navigation & Stability

  • Support participants with housing navigation, including applications, referrals, landlord coordination, and housing queue management.
  • Effectively manage and monitor HMIS and housing waitlists.
  • Support transitions to permanent housing in coordination with Continuum of Care (CoC), behavioral health providers, and partner agencies.

Behavioral Health & Interagency Coordination

  • Coordinate services with behavioral health providers, substance use treatment programs, and other service partners.
  • Support participant engagement in behavioral health services as required under the BHBH program.
  • Participate in multidisciplinary case conferencing and warm handoffs.
  • Ensure level of care assessments and endure appropriate placement

Documentation, HMIS & Compliance (AB 977)

  • Maintain accurate, timely, and complete documentation in participant files and HMIS, including assessments, case notes, service plans, and outcome tracking.
  • Ensure documentation meets DHCS, AB 977, and BHBH reporting requirements.
  • Submit monthly caseload reports to the Director of Operations.

Transitional Living Operations & Safety

  • Coordinate with Transitional Living House Monitors to: o Facilitate weekly house meetings
  • Assign and monitor daily and weekly chores oConduct facility inspections for safety and cleanliness
  • Provide conflict resolution and crisis intervention.
  • Administer and interpret random drug and alcohol screens.
  • Enforce Transitional Living and shelter rules in a consistent, non punitive manner.
  • Respond to emergency situations as needed.

Financial & Program Administration

  • Assess, calculate, collect, and document Transitional Living program fees.
  • Remit participant payments to the Executive Director in a timely manner.
  • Coordinate group activities including community service and program events.

Additional Duties

  • Collaborate with P.A.T.H. programs and external agencies to meet participant needs.
  • Perform other duties as assigned in support of the organization’s mission.

Qualifications

  • Knowledge or experience working with individuals with mental illness, substance use disorders, and/or chronic homelessness.
  • Understanding Housing First, trauma informed care, and harm reduction principles.
  • Strong organizational, communication, and documentation skills.
  • Demonstrated ability to manage crises and de escalate conflict.
  • Ability to work independently, prioritize tasks, and function well under pressure.
  • Computer proficiency, i.e. Microsoft Office 365, with willingness to learn HMIS.
  • Willingness to complete First Aid/CPR training after hire.
  • Reliable transportation, valid California Driver’s License, and proof of insurance.

Requirements

  • Must be 18 years of age or older
  • Valid California Driver’s License

Physical & Working Conditions

While performing job duties, the employee must be able to:

  • Stand for extended periods
  • Frequently stoop, kneel, bend, crouch, and reach
  • Lift, push, and pull up to 50 lbs routinely and 75 lbs occasionally
  • Operate computer and office equipment
