Position Summary:
Pomerene Hospital is seeking a dedicated and patient‑focused Inpatient Hospital Pharmacist to join our clinical team. As a critical member of a small rural hospital, you will play a hands‑on role in ensuring safe, effective, and compassionate medication management for our inpatient population. This position offers the opportunity to practice at the top of your license, collaborate closely with providers and nurses, and make a meaningful impact on patient care in a close‑knit community setting.
Key Responsibilities:
- Provide comprehensive clinical pharmacy services for inpatient units, including medication order verification, therapeutic drug monitoring, and clinical consults.
- Review and interpret provider orders to ensure safe, evidence‑based, and cost‑effective medication use.
- Participate in daily interdisciplinary rounds and collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes.
- Prepare, dispense, and oversee the safe handling of medications, including sterile and non‑sterile products.
- Monitor medication therapy for efficacy, safety, interactions, and appropriate dosing, especially for high‑risk medications.
- Provide patient education on medication therapy, discharge prescriptions, and medication reconciliation.
- Support antimicrobial stewardship initiatives and other quality‑improvement programs.
- Maintain accurate documentation in the electronic health record.
- Ensure compliance with hospital policies, state and federal regulations, and accreditation standards.
- Serve as a medication‑related resource for clinical staff and participate in staff education.
- Contribute to on‑call or weekend coverage as needed.
Qualifications
- Education:
- RPh or PharmD; PharmD preferred.
- Licensure:
- Current and active pharmacist license in the State of Ohio (or eligibility for licensure).
- Experience:
- Hospital or clinical pharmacy experience preferred; new graduates with strong clinical training will be considered.
- Skills & Competencies:
- Strong clinical judgment and problem‑solving skills.
- Excellent communication and collaboration abilities.
- Comfort working in a rural hospital environment with broad clinical responsibilities.
