An exciting career awaits you at Pomerene. We are looking for a LPN to join our Case Management team. Working hours for this position are 7am - 5:30pm.
The Nurse Case Manager is responsible for the coordination of care and comprehensive management for patients who have chronic conditions with the goal of improving healthcare outcomes. This is done through assessments, care planning, overall evaluation of individual care needs, support and guidance on preventative services, education, referrals as needed to specialists and community resources by utilizing telephonic communications.
Essential Functions
- Moves throughout the facility, appropriately assists in emergency drills and performs the physical requirements needed to deliver services as assigned.
- Performs LPN Case Management duties under the direction of the Practice Administrator and/or Office Manager according to hospital and department policy and procedure.
- Maintains quality and compliance according to national and state guidelines.
- Demonstrates commitment to Pomerene Hospital's Values.
- Delivers services that builds customer satisfaction.
