JOB

The Community Health Manager provides strategic leadership for the department's community health initiatives, public health policy efforts, and emergency preparedness functions. This position manages a team of planners and preparedness staff to advance department-wide priorities, strengthen community partnerships, and ensure the development, implementation, and evaluation of the Community Health Improvement Plan (CHIP). The Manager uses data, evidence, and collaborative engagement to shape strategies that improve community conditions and advance health and equity across Polk County.

EXAMPLE OF DUTIES

Lead the development, implementation, monitoring, and reporting of the Community Health Improvement Plan (CHIP) in alignment with CHA findings, County priorities, and PHAB accreditation standards.Manage a team of Community Health Planners to ensure effective planning, facilitation, and evaluation of community health strategies.Oversee the emergency preparedness program, including staff supervision, workplan development, grant deliverables, and maintenance of plans, trainings, and exercises.Ensure alignment between community health planning, emergency preparedness, and broader departmental priorities (strategic plan, PMQI, performance metrics).Integrate data and evidence-based practices into program design, goal setting, evaluations, and communication of outcomes.Serve as the primary departmental lead for community relations and external partnerships that advance public health goals.Build and maintain strong relationships with community organizations, coalitions, local governments, school districts, health systems, and residents.Ensure meaningful community participation in the CHIP and related initiatives, using inclusive and equitable engagement strategies.Represent the department at meetings, coalitions, advisory groups, and public forums; may serve as spokesperson when delegated.Support the Director in leading the department's public health policy activities, including policy research, analysis, development, and evaluation.Monitor local, state, and federal policy developments that affect community health; prepare briefs, recommendations, and impact summaries.Collaborate with internal teams and external partners to advance policies that improve community conditions, reduce inequities, and strengthen population health.Support Health in All Policies (HiAP) approaches in Polk County government to integrate health considerations into decision-making.Work with communicable disease and surveillance teams to interpret data, identify emerging trends, and translate findings into strategic action.Develop and track performance measures for CHIP priorities, community partnerships, and policy initiatives.Prepare reports, presentations, dashboards, and communication materials for internal leadership, the Board of Supervisors, partners, and the public.Provide direct supervision, coaching, development, and performance management of Community Health Planners and Preparedness staff.Foster a positive culture grounded in accountability, continuous improvement, and cross-team collaboration.Develop staff capacity in community engagement, policy analysis, facilitation, and project management.Oversee grant management, work plans, after-action reports, and compliance with state and federal emergency preparedness requirements.Support response activities during emergencies or public health incidents; may serve in incident command roles as assigned.Support the fiscal agent responsibilities of the PHEP, CRI, and HPP grants.Performs related duties as required.

SUPPLEMENTAL INFORMATION

Bachelor's degree in Public Health, Public Administration, Community Health, Emergency Management, Social Work, or related field.5 years of progressively responsible experience in community health, public health planning, policy, emergency preparedness, or a related area.2 years of supervisory or team leadership experience.Master's degree in public health (MPH), Public Administration (MPA), Emergency Management, or related field is preferred.Special Requirements: Candidates must be able to successfully pass relevant CDC courses within six months of hire and maintain related training annually. HIPAA training is required upon hire and annually. National Incident Management System (NIMS) training is required within six months of hire.A criminal background check is required.A drug screen is required prior to final offer of employment.A valid State of Iowa driver's license is required. In case of a public health or community emergency, must be willing and able to respond to after-normal workday or irregular and on-call emergency requests on nights, weekends and holidays.
