Position Summary
The Disaster Case Manager serves as a primary point of contact for individuals and families in Polk County impacted by natural disasters, guiding them through the complex recovery process. This full-time position requires a compassionate, self-directed, and resourceful individual who will work with disaster survivors to develop and implement realistic long-term recovery plans. The Disaster Case Manager will coordinate with the Polk County Long Term Recovery Group (LTRG), partner agencies, and other resources to assist survivors in addressing unmet disaster-related needs, rebuilding their lives, and achieving a stable recovery.

Key Responsibilities

  • Maintain an active caseload of disaster survivors by providing comprehensive, individualized case management services.
  • Serve as the primary point of contact for survivors, building trust-based relationships and acting as an advocate throughout their recovery journey.
  • Conduct initial and ongoing needs assessments to verify disaster-related impacts and identify unmet needs.
  • Develop and implement a personalized long-term recovery plan with each survivor, monitoring progress and adjusting the plan as needed.
  • Facilitate access to and coordinate resources, including financial assistance, rebuilding programs, and support services, while preventing duplication of benefits.
  • Serve as a liaison between survivors and government agencies (e.g., FEMA, SBA), volunteer organizations, and local non-profits.
  • Prepare and present individual cases to the LTRG's Unmet Needs Committee to secure additional resources for clients.
  • Document all case progress, communications, and outcomes accurately in accordance with the LTRG's protocols.
  • Stay informed on available community resources and support systems to provide accurate and effective referrals.
  • Ensure the timely closure of cases and, when appropriate, conduct survivor satisfaction interviews.
  • Protect all confidential client information with the highest level of respect and integrity.
  • Participate in relevant LTRG meetings, training sessions, and coordination calls.

Qualifications and Skills

  • Experience in case management, social work, or a related field; disaster-specific case management experience is preferred.
  • Strong interpersonal, communication, and organizational skills.
  • Exceptional empathy and the ability to work respectfully with people from diverse backgrounds who are experiencing trauma.
  • Proficiency in computer skills, including Microsoft Office Suite and experience with case management or database systems.
  • Ability to work independently, manage a demanding caseload, and navigate a fast-paced environment.
  • A resourceful and creative problem-solver, with excellent attention to detail.
  • Ability to maintain professional boundaries and respect client privacy.
  • A valid driver's license, reliable transportation, and proof of adequate insurance are required for home visits and meetings.
  • Willingness to work occasional evenings or weekends as needed during periods of high demand.

Working Conditions

  • This is a full-time position requiring 40 hours per week.
  • Work will be performed both in an office setting and in the field, with some local travel required for home visits, meetings, and coordination efforts.
  • The position is grant-funded and its continuation is contingent upon funding availability.

Pay: $45,000.00 - $55,000.00 per year
