What You’ll Need:
- Minimum age: 18
- Bachelor’s degree OR RN licensure with 1 year of experience working with individuals with disabilities
OR Bachelor’s in unrelated field with 2 years of relevant experience. - Strong communication, organization, and problem-solving skills.
- Proficiency in Microsoft Office and Therap.
- Valid driver’s license and Social Security number
- Ability to type 70 WPM and navigate basic computer systems
- Basic computer and documentation skills
- Ability to pass required background and reference checks
About the Role:
The Case Manager is responsible for assessing the needs, goals, and preferences of individuals with disabilities and special needs, advocating on their behalf, and ensuring they are connected to the services and supports they need to thrive. This role works under the general supervision of the Director of Individual and Family Support and requires strong coordination, communication, and problem‑solving skills. Your responsibilities may include:
- Conduct annual and ongoing assessments to evaluate individual needs and service effectiveness.
- Develop and update individualized support plans in collaboration with the individual and their support team.
- Connect individuals to appropriate community resources and service providers.
- Monitor services for quality, satisfaction, compliance, and funding requirements.
- Advocate for individual rights, informed choice, and independence.
- Collaborate with families, providers, community partners, and internal teams to ensure coordinated care.
- Maintain accurate, timely documentation in accordance with compliance and regulatory standards.
- Uphold confidentiality, ethical standards, and professional boundaries at all times.
Travel Requirements: This position requires frequent local travel throughout the Upstate region. Case Management services are provided across 12 Upstate counties, not limited to Spartanburg County.
- Travel may occur several days per week.
- Some days may require being on the road for the entire workday, depending on visit schedules and monthly requirements.
- Visits occur in personal homes, residential provider locations, day programs, and occasionally hospitals, schools, or detention facilities.
- A reliable personal vehicle is required.

- Strong critical thinking and problem-solving skills
- Excellent time management and organizational ability
- Independent work with minimal supervision
- Strict adherence to deadlines and non-flexible due dates
- High-level documentation accuracy and attention to detail
Physical Requirements:
This role involves regular standing, walking, lifting (up to 25 lbs), and hands-on support. Reasonable accommodations are available.
