Position Summary
The Re-Housing Specialist will provide case management and housing support to families in shelter. This role conducts assessments, develops housing plans, and assists participants in securing and maintaining stable housing. This is a hands-on, in-person role with meaningful impact on the lives of families in our community.
What You’ll Do
- Conduct intakes, assessments, and goal planning with shelter participants
- Provide ongoing case management through individual and group support
- Maintain accurate and timely documentation in EOHLC program systems
- Assist participants with housing search, applications, and placement
- Develop and maintain landlord relationships; act as liaison as needed
- Connect participants to employment, education, and supportive services
- Participate in case conferencing with internal and external providers
- Support implementation of re-housing and stabilization plans
- Enforce shelter rules and program policies
- Perform other duties as assigned
Qualifications
- Bachelor’s degree in Social Work or related field preferred; equivalent experience accepted
- 2–5 years of experience in direct care, housing, or related services preferred
- Experience working with homeless or low-income populations preferred
- Valid driver’s license required
Skills & Knowledge
- Strong communication and organizational skills
- Ability to manage multiple cases and priorities
- Knowledge of housing resources and social service systems
- Proficiency with case management software and documentation
Work Schedule
- 32–40 hours per week; 8-hour shifts
- Weekday, evening, and weekend openings
- Rotating on-call schedule
Physical Requirements
- Primarily in-person work
- Ability to sit or stand for extended periods
- Regular use of computer, phone, and office equipment
