We are seeking a dedicated, caring Housing Specialist professional who enjoys working in the community with our Housing team to ensure services and support to residents on-site in our Brockton area locations.
What You'll Do
- Review the needs of residents to link the community programs and services
- Serve as a multi-disciplinary assessment team leader and meet with the team to assure service coordination
- Facilitate care conferences with residents, family, and other participants.
- Plan and coordinate social, educational, and recreational programs for residents
- Prepare and distribute a newsletter to keep residents informed of planned activities
- Your passion for helping others and desire to make a difference.
- Adaptability to be able to change as the needs of consumers and/or programs change
- At least two years of experience in the human services field, preferably with the elder population
- Bachelor's degree in social work or related field preferred
- Excellent written and verbal skills.
- Excellent computer skills.
- Proficient with Microsoft Office Suite or related software.
- Bilingual a plus; Haitian Creole or Cape Verde Creole is preferred
