What You’ll Do
- Manage and staff active patient cases ensuring zero-gap coverage
- Match caregivers to patients based on skills, compatibility, compliance, and location
- Respond to call-outs, emergencies, schedule changes, and service disruptions in real time
- Communicate clearly and compassionately with caregivers, patients, families, care managers, and clinical teams
- Use Caribou / HHA Exchange and internal tracking systems to maintain accurate case assignments and documentation
- Proactively follow up on caregiver availability, confirmations, and schedule changes
- Flag risk trends (frequent call-outs, recurring issues, difficult-to-service cases) and escalate appropriately
- Collaborate closely with Recruitment, Compliance, Payroll, Nursing, and HR to ensure caregivers remain eligible and cases remain stable
- Document all actions clearly, because what isn’t documented didn’t happen

Qualifications
- Experience in home care coordination, staffing, or healthcare scheduling (required)

- Tech-savvy: scheduling platforms, spreadsheets, internal trackers
- Ability to multitask, prioritize, and work through high-volume demands
- Bilingual (English/Spanish) strongly preferred
