Responsibilities
- Perform patient visits and consultations regarding home health services and end-of-life options.
- Coordinate care needs, including medication delivery, medical supplies, and durable medical equipment.
- Maintain accurate clinical records in compliance with policies, regulations, and accreditation standards.
- Assess referral appropriateness, process admissions, and ensure proper documentation of all patient interactions.
- Communicate scope of care effectively to patients, families, and healthcare teams.
- Licensed Registered Nurse (RN) in Illinois with active license.
- Basic Life Support (BLS) certification within 30 days.
- Minimum of 1 year of nursing experience.
- Valid driver’s license and proof of auto insurance.
- Strong clinical documentation skills, patient assessment, and care coordination abilities.
