We are seeking a Nurse Practitioner to join a Home-Based Primary Care Program serving high-needs, medically complex, and geriatric patients in various Maryland locations.

- Providing comprehensive primary care for homebound patients, evaluating, diagnosing, and treating medical conditions.
- Collaborating with a multidisciplinary team and participating in weekly meetings.
- Coordinating patient care with specialists and team members.
- Traveling to patient homes using a personal vehicle.
- Engaging in administrative tasks and maintaining licensure and certifications.
- Maryland NP licensure and CPR certification.
- 3 years of inpatient or outpatient practice experience, preferably in geriatrics or high-acuity care.
- Experience in primary, urgent, or acute care preferred; palliative or geriatric certification advantageous.
- Ability to work independently with excellent communication skills.
