Qualifications & Skills
- Active LPN license in the state of practice
- Minimum of two years of LPN experience, hospice experience preferred
- Current driver’s license with ability to spend approximately 20% of time traveling
- Deliver compassionate care supporting patient comfort and dignity
- Provide after-hours assessments and support
- Reinforce end-of-life care plans with patients and caregivers
- Maintain accurate EMR documentation
- Travel to patient homes within assigned territory
