Role Overview: The Geriatric Nurse Specialist provides comprehensive, patient-centered care to older adults, supporting approximately 1,600 Medicare patients across diverse settings including primary care practices, homes, and senior living communities.
Key Responsibilities: Conducts preventive services such as Wellness Visits, screenings, immunizations, and assessments for cognitive, mobility, and social needs. Facilitates care coordination, medication reconciliation, and advance care planning. Performs home visits, evaluates home safety, and collaborates with interdisciplinary teams and community partners to ensure seamless transitions and high-quality care.
Qualifications & Skills: Licensed RN or LPN with 10 years of geriatric experience, strong clinical knowledge, excellent communication, and organizational skills. Valid licenses for MA and NH preferred. Experience with Medicare, care management, and senior living settings is advantageous. Must demonstrate compassion, teamwork, and ability to travel.
