- Providing FCT clinical services in clients' homes and communities, ensuring model fidelity and completing required documentation.
- Developing and coordinating service plans with Treatment Teams, conducting assessments, and facilitating family engagement through psychoeducation and intervention.
- Offering 24/7 on-call support, assisting families with goal setting, discharge planning, and accessing community resources.
- Collaborating with medical and non-medical providers to ensure coordinated care.
- Master's Degree in Social Work, Counseling, or related field.
- At least two years of experience working with youth or families.
- FCT certification preferred; must complete certification within one year if not already certified.
- State-specific licensure or credentialing requirements.
- Strong communication, collaboration, and organizational skills.
