Responsibilities:
- Evaluate patients, develop individualized treatment plans, establish goals, and progress care through discharge and follow-up.
- Complete accurate, timely clinical documentation and communicate patient progress with physicians and other healthcare providers.
- Provide compassionate, service-oriented care while maintaining strong communication with patients, staff, and referral sources.
- Supervise rehabilitation aides, administrative staff, and PTAs as applicable and in accordance with state practice acts.
- Build relationships with referral sources and support community outreach through events, presentations, screenings, and education.
- Participate in continuing education, staff development, and clinical or program leadership opportunities as needed.
Qualifications:
- Degree from an accredited Physical Therapy program.
- Current Physical Therapist license and CPR certification, maintained as required.
- Complete all required compliance standards.
