Job Summary
We are seeking a dedicated and detail-oriented MDS Coordinator to oversee the comprehensive assessment and documentation process within skilled nursing, inpatient rehabilitation, or long-term care facilities. The ideal candidate will possess a strong background in clinical documentation, medical coding, and healthcare management, with expertise in utilizing electronic health record (EHR) system (Point Click Care). As an MDS Coordinator, you will play a vital role in ensuring compliance with regulatory standards, optimizing patient care documentation, and supporting clinical teams to deliver high-quality services. Responsibilities
- Conduct thorough assessments of patient records to accurately complete the Minimum Data Set (MDS) in accordance with CMS regulations and NCQA standards.
- Collaborate with interdisciplinary teams including nursing, therapy, and medical staff to gather relevant clinical information for documentation review.
- Ensure compliance with HIPAA regulations by maintaining confidentiality and security of all medical records and documentation.
- Utilize EMR/EHR systems such as Epic, Cerner, or Athenahealth to input data, track progress, and generate reports related to patient care and documentation accuracy.
- Perform utilization review and management activities by analyzing clinical data to support appropriate level of care decisions across inpatient, outpatient, home care, or hospice settings.
- Apply CPT coding, ICD-9/ICD-10 coding standards, and DRG classifications to facilitate accurate billing and reimbursement processes.
- Assist in discharge planning by coordinating with case management teams to ensure seamless transitions of patient care between facilities or settings.
- Maintain knowledge of Medicare requirements, managed care policies, and regulatory updates affecting long-term care documentation practices.
Skills
- Proficiency in EMR/EHR systems Point Click Care.
- Strong understanding of medical terminology, anatomy, physiology, and clinical documentation improvement strategies.
- Knowledge of ICD coding (ICD-9/ICD-10), CPT coding, DRG assignment, and healthcare reimbursement methodologies including Medicare policies.
- Experience with utilization review/management within acute care or hospital environments;
- Background in nursing or health information management with familiarity in medical records review and compliance standards such as NCQA standards.
- Excellent organizational skills with the ability to manage multiple priorities efficiently while maintaining attention to detail.
- Effective communication skills for collaborating across multidisciplinary teams and providing education on documentation best practices.
