What are the responsibilities and job description for the Transitions/Move-In Coordinator position at PIPER SHORES?
GENERAL SUMMARY:
As part of the move-in process, this role works to ensure that Independent Living residents have full access and awareness of the scope of services available to them at Piper Shores and within the broader community. Serves as a key liaison between residents, families and clinical staff to ensure appropriate transitions of care and services. Along with the Transitions Team, coordinates transitions from Independent Living to Assisted Living or Memory Care.
RESPONSIBILITIES:
Independent Living
- Serves as the primary contact for new and current residents, providing concierge-style assistance regarding wellness services, community resources, and support programs.
- Develops and maintains new resident orientation checklists and ensures that all new residents follow a comprehensive and consistent process.
- Orients all new residents to the community, including policy review & sign-off and ensuring completion of new resident orientation.
Assisted Living/Memory Care Transitions
- Orients all new Assisted Living residents to Holbrook, including policy review & sign-off and ensuring completion of new resident orientation.
- Provides guidance to family members through transitions from Independent Living to Assisted Living or Memory Care.
- Maintains regular and on-going relationships with residents and families to discuss needs or concerns and mediates issues that may arise between resident/family/staff.
- Supports the assisted living admissions process, including inquiries and tours. Will facilitate or arrange for supportive educational programs for independent living residents and their families.
Other
- Serves as the front-line service representative for Independent and Assisted Living Residents directing resident concerns to the appropriate department head and monitors concerns to resolution with department heads.
- Provides admissions functions through the Interdisciplinary Team utilization review, and Transition Team process; facilitates transitions teams meetings.
Qualifications:
- Bachelor's Degree or associate’s degree in Communications, Marketing, Hospitality, Social Services or related field preferred.
- Minimum of 3-5 years of work experience in a professional setting required.
- Concierge and service-oriented approach to building relationships; proactively seeks out new connections and easily develops rapport with others.
- Passion for working with older adults and an appreciation of the aging process.
- Demonstrated ability to work independently and collaboratively.
- Must be able to maintain confidentiality at all times.
- Must be highly organized and able to balance multiple priorities within the time parameters of the role.
- Strong verbal and written communication skills.
- Must be able to maintain confidentiality at all times.
- Knowledge of MS Office Suite, ability to learn new technologies as needed.
