Monday-Friday, 8a-4p, 40Hrs/week
(Hours may vary), Requires rotation 
of on call      


POSITION SUMMARY
The focus of the client care coordinator is to support adults with Intellectual Disabilities that live within Pioneer’s 24-hour CILA (residential) group homes. The coordinator serves as the primary advocate for each client on their caseload, ensuring not only compliance, but quality of life and overall satisfaction within all programs. The QIDP provides necessary services and supports to ensure client’s progress towards goals they develop through their annual Person-Centered Planning.

Essential Functions Statement(s)

•    Maintains a caseload of individuals with intellectual and developmental disabilities receiving CILA services within DHS regulations. 
•    Serves as primary point of contact and advocate for client care connecting team to medical, behavioral, mental health, day/residential services, and other third-party agencies as necessary. This includes but is not limited to ongoing person-centered planning, admission/discharge, IDT meetings, crisis situations, and satisfaction of services. 
•    Responsible for completing monthly/yearly documentation including the Implementation Strategies, monthly service reviews, assessments, releases/consents, critical incident reports, CIRAS, surveys, progress notes, and billing. All required information is entered into the agency’s Electronic Health Record system, where and when appropriate.
•    Follow the SMART module when writing goals by keeping them specific, measurable, achievable, realistic, and time oriented. 
•    Maintains accountability for initial and ongoing training of the Implementation Strategies and goal data tracking sheets across programs. 
•    Prepare and present all necessary restrictions to the Human Rights Committee (HRC)
•    Attend psychiatric and neurological appointments as necessary for increased behavioral or dementia related concerns.
•    Ensures compliance with auditors such as BQM (Bureau of Quality Management), BALC (Bureau of Accreditation, Licensure, and Certification), CARF (Commission on Accreditation of Rehabilitation Facilities), Illinois DHS (Department of Human Services, HCBS (HOME AND COMMUNITY BASED SERVICES) Rule, etc. Assist in audits, as necessary. 
•    Supports client in providing the opportunity for integration and access to the community, choice, individual rights, an independence in accordance with the HCBS Rules 
•    Ensure accuracy of nightly attendance and submission to billing for reimbursement monthly. 
•    Participants in an on-call rotation for crisis calls/hospital visits, as necessary.
•    Conducts at least biweekly visit with each assigned client at CDS and CILA. 
•    Conducts check-in visits with clients while in hospital and in rehab and remains an advocate for care. 
•    Attend staff meetings and resident council meetings monthly in CDS and CILA.
•    Communicates staff performance concerns with assigned House Supervisor. 
•    Maintain regular, reliable, and predictable attendance.
•    Must possess excellent verbal and written communication skills.
•    Conducts all phases of client services in compliance with the laws on confidentiality and HIPAA.  
•    Performs all other case management duties as assigned by Supervisor.


Provided Training 
•    Computer Systems: SmartCare (Electronic Health Record System), Teams (Communication platform), Policy Tech (Compliance documents/forms), Relias (Agency Training)
•    Direct Service Professional Training. (Required before becoming a QIDP)
•    QIDP certification (completed in house with correct eligibility)
•    DHS training on Rule 115, 116, 120 as necessary
•    Assessments-ICAP, safety assessments, satisfaction survey, dementia tool, etc. 
•    CIRAS (Critical Incident Reporting and Analysis System) reporting and ICAP redeterminations. 
•    Other training courses include Safety Care, CPR, Hearthstone, etc.

SKILLS & ABILITIES

Education:    Bachelor's Degree (four-year college or technical school) Required, Field of Study: Human Service can include but not limited to sociology, special education, psychology, criminal justice.  Proof of education must be provided. 

Experience:    Must satisfy all requirements to be recognized by DHS as a QIDP, including 1 year of experience working directly with people that have developmental disabilities. Proof of experience must be provided. (Candidates with at least 6 months of experience may qualify for a case manager position prior to QIDP eligibility)


Computer Skills:    Microsoft Office, Word, Excel, PowerPoint, Outlook, Teams, SmartCare

Certifications &
Licenses:    QIDP Certification or eligible to become a QIDP following Department of Human Services standards.  
 

Other Requirements:    Must have and maintain a valid driver's license. 

Maintains 12 CEUs annually as required to uphold QIDP certification once obtained.


Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits:
$25,000 bodily injury, each person
$50,000 bodily injury, each accident
$25,000 property damage

Ability to use Phone, Fax, Copier, and file required.
