Job Summary:

We are seeking a dynamic and highly motivated licensed Charge Nurse (RN/LPN) to lead our second shift team at Pinnacle Health & Rehab at Canton. As a Charge Nurse, you will be responsible for oversight of a team of certified nursing assistants, collaborating with direct care workers and all ancillary departments. With a warm and caring environment, we offer skilled nursing/rehabilitation and long-term care to our patients.

Responsibilities:

• Provide leadership and oversight to the clinical team, ensuring high-quality patient care and adherence to facility policies and procedures

• Collaborate with direct care workers and ancillary departments to ensure seamless coordination of patient care

• Develop and implement individualized care plans for patients, ensuring patient-centered care and maximizing patient outcomes

• Participate in quality improvement initiatives and committees to enhance patient care and facility operations

• Maintain accurate and timely records, reports, and statistics

• Supervise and mentor staff, providing guidance and support to ensure excellent patient care

Requirements:

• Current Maine nursing license

• Experience in Long Term Care is a plus, but not required

• New graduates invited to apply

• Vaccinations must be up-to-date, and proof provided before hire

Part-time day and evening positions also available.
