Job Summary:

Responsibilities:
- Oversee resident care plans and review medical history for coordination and completion of MDS
- Responsible for conducting care plan meetings and ensuring that all necessary documentation is completed in a timely manner
- Collaborate with healthcare professionals to ensure that resident needs are being met and that care plans are being implemented effectively
- Develop and maintain accurate and comprehensive records of resident care plans and progress
- Participate in quality improvement initiatives to ensure that high-quality care is being provided to residents
- Provide education and training to healthcare professionals on MDS and care planning principles
- Must have at least two years of experience completing MDS and be MDS certified (Preferably RAC certified)
- Must be a Maine licensed RN with Long Term Care experience
- Bachelor's degree in Nursing or related field
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information with discretion
- $80,000 ANNUALLY competitive salary
