Job Summary:

Executive Search Network/Platinum Staffing is seeking a highly skilled and experienced MDS Coordinator to join a prominent skilled nursing facility in Southern Maine. As a key member of the healthcare team, you will be responsible for overseeing resident care plans and reviewing medical history for coordination and completion of MDS, care plan meetings, and other related tasks.

Responsibilities:

• Oversee resident care plans and review medical history for coordination and completion of MDS
• Responsible for conducting care plan meetings and ensuring that all necessary documentation is completed in a timely manner
• Collaborate with healthcare professionals to ensure that resident needs are being met and that care plans are being implemented effectively
• Develop and maintain accurate and comprehensive records of resident care plans and progress
• Participate in quality improvement initiatives to ensure that high-quality care is being provided to residents
• Provide education and training to healthcare professionals on MDS and care planning principles
  
  
  

Requirements:

• Must have at least two years of experience completing MDS and be MDS certified (Preferably RAC certified)
• Must be a Maine licensed RN with Long Term Care experience
• Bachelor's degree in Nursing or related field
• Ability to work in a fast-paced environment and prioritize tasks effectively
• Strong communication and interpersonal skills
• Ability to maintain confidentiality and handle sensitive information with discretion
