!Our model is built around you having time to practice medicine. With dedicated care coordinators handling administrative work and RNs managing triage, you can focus on what you do best: comprehensive clinical care. Our collaborative team structure provides robust support for chart prep, patient education, and follow-up management, creating a workflow that enhances both provider satisfaction and patient outcomes

• oProvide comprehensive onsite primary care to seniors in assisted living and senior living communitie

• nPartner with our Medical Director to refine clinical policies, protocols, and guidelines as our practice evolve
• sDeliver monthly Chronic Care Management (CCM) visits and oversee CCM documentatio
• nCollaborate with senior living leadership and care teams to optimize care plan execution and patient outcome

The base salary range for this role is $135,000 to $160,000 plus generous bonus, equity, and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data.
