We are seeking a dedicated and compassionate full-time Family Nurse Practitioner to join our multidisciplinary team in providing professional medical services for AACI Health Center patients.
As Part of Our Team, You Will:

Provide comprehensive primary care services, including assessment, screenings, diagnosis, treatment, and preventive health services.

Conduct physical examinations, obtain medical histories, and develop appropriate treatment plans.

Prescribe and manage medications, develop individualized treatment plans, and monitor patient outcomes in collaboration with the care team

Educate patients on disease prevention, medication adherence, and overall wellness.

Collaborate with physicians, behavioral health providers, and interdisciplinary teams to ensure coordinated, high-quality care

Maintain accurate, timely, and complete documentation in the electronic health record (EHR).

Coordinate referrals and consultations with specialists to support comprehensive patient care

Support team-based operations, including meetings, cross-coverage for clinicians, and participation in after-hours on-call rotation
Qualifications
- Master’s Degree or Doctorate from an accredited Nurse Practitioner program

Current, unrestricted License as a Nurse Practitioner in the State of California

Board Certification from the California Board of Registered Nursing

Active and unrestricted DEA license

Current BLS certification
-
Minimum 2 years of Primary Care clinical work experience required

Experience working in a community health center, FQHC, or underserved healthcare setting preferred.
