Position Summary
The Care Team Lead is responsible for overseeing daily care operations, supporting care staff, fostering great working relationships between staff members, and ensuring high-quality, person-centered services. This role serves as a bridge between frontline caregivers and management, helping maintain a positive, efficient, and compliant care environment.

Key Responsibilities
- Lead, support, and mentor care team members in delivering high-quality care
- Coordinate daily schedules, assignments, and staffing needs
- Monitor care documentation for accuracy and compliance
- Assist with onboarding and training new team members
- Communicate with families, residents/clients, and interdisciplinary teams
- Support quality improvement initiatives and service plans
- Address concerns, escalate issues appropriately, and help resolve conflicts
- Ensure adherence to organizational policies, safety standards, and regulatory requirements
- Participate in meetings, reporting, and performance evaluations as needed
Qualifications
- High school diploma or equivalent
- Prior experience in caregiving or care coordination
- Leadership or supervisory experience strongly preferred
- Strong communication, organization, and problem-solving skills
- Ability to work collaboratively in a fast-paced care environment
- Knowledge of care standards, privacy laws, and best practices (HIPAA, state regulations)
- Certifications (CNA, CMA, HHA, MA, or equivalent) a plus

- Education: We will send you to additional certifications as needed to grow in your position (paid for by Pine Brook Pointe with a one year employment commitment), we provide monthly in-services to help further education in this field. In services may include: dementia care, resident rights, etc.
- Training: We provide on the job training, and ensure you have all the knowledge and resources before having you work on your own. Once you are working "on your own" you won't ever actually be alone. There are always other caregivers in the building, and we all work as a team to make sure our residents are getting the best care possible.
