Grace Unlimited Home Health Care is seeking experienced PRN RN Case Managers to provide high-quality home healthcare to clients primarily in the Fort Worth area, with coverage in surrounding Dallas–Fort Worth communities as needed. Role Responsibilities:
Provide professional, compassionate nursing care in clients’ homes
Perform assessments and manage individualized care plans
Accurately document care using OASIS/Kinnser
Manage diverse patient needs in a home health setting
Respond to PRN and urgent visit needs as assigned
Qualifications:
Graduate of an accredited nursing program
Active Texas RN License
Valid Driver’s License and reliable transportation
Minimum 1 year of home health experience
Experience with OASIS/Kinnser preferred
Ability to travel locally within the Fort Worth area
Flexible availability for PRN visits
Work Location:
Client homes throughout the Denton area
Field-based / on-the-road role
