Education: High School diploma or an equivalent combination of education and experience.
Required: Excellent communication skills. Two to three years of care coordination experience in a health care setting. Prior management experience of 2 years or more years.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment
Salary Range: $52,806 - $70,107

Job Duties/Responsibilities -
- Supervision of member care coordinators
- Training and development of administrative staff
- Works in collaboration with management to ensure PHS policy compliance.
- In collaboration with management team insures optimal patient access and efficiency practices are met.
- Coordinates monthly member care coordinator meetings and creates meeting agenda and minutes.
- Attends payor and network meetings as needed.
- Assist care management teams, internal and external for the coordination of patient care.
- Monitors and reviews member care coordinator productivity and key performance indicators.
- Supports member care coordinators with oversight and guidance as needed.
- Analyze and synthesize population health outreach data to optimize and guide the team's ongoing community interventions.
- Assists with interviews and hiring practices for the care coordination team.
- Engages in patient outreach to identify social determinants of health needs, linkage to services and resources, care coordination, and referral coordination.
- Assists Population Health Leadership in the execution of population health initiatives as needed.
