What are the responsibilities and job description for the Care Coordinator position at Victorian Senior Care?
Overview
Assure Resident Care Services are provided in compliance with Victorian Senior Care Policy, DMA Clinical Coverage Policy 3L and NC DHSR Licensure requirements. Have a working knowledge of Adult Care Home Policies and Procedures and NC DHSR requirement for the operation of an Adult Care Home. Reports to the Executive Director/Administrator.

Responsibilities
Oversight of Personal Care Service and Personal Care Service Documentation.
Train staff about personal care services, resident needs and ensuring staff is providing the personal care services required of each resident based on the current service plan/care plan in place, and as indicated on aide task worksheets.
Oversight of Medication Administration and Medication Administration Records.
Medication Administration training per NC Adult Care Home Rules and Regulations and VSC Policy and Procedures.
Coordinates and follow up on all Licensed Health Professional Support Task, Pharmacy Reviews, Physician Referrals, or other outside referral agencies.
Assure FL2, Care Plans, physician orders and other applicable documents are current and up to date based on residents’ current needs.
Monitor skin care, nail care, vitals, intake and output of residents.
Supervise direct care staff and medication aides while coaching, mentoring and training them with hands on approach. Provides training, supervision, and monitoring of associates in the administration of direct care within the community; provides hands on resident care as needed or warranted
Will be responsible for rotating on-call. (May Include some nights, weekends, and holidays)
Continuously evaluate the Resident Care Program by utilizing quality enhancement tools and customer feedback, which may necessitate improving the program to meet the ever-changing needs of the residents.
Support and actively participate in census building initiatives: event hosting, external and internal program presentations as requested by management.
Maintain a positive approach and relationship with resident’s families/responsible parties. Provide support and guidance on assisted living care and the processes of placement. Link families/responsible parties to support groups and outside resources when needed.
Perform other LPN/RN Duties that are allowed per NC Board of Nursing.
Be familiar with Residents' Bill of Rights and assist residents in exercising those rights.
Notify the ED of any concerns of neglect/abuse/exploitation by other staff.
Follow all regulations as required by Federal HIPPA Laws.
Perform other duties as required by circumstances.
Qualifications
NC Licensed Practical Nurse or NC Licensed Registered Nurse in good standing with the NC Board of Nursing
Must have a current, unencumbered license in North Carolina and with two years of clinical or direct patient care experience working in a health care, home care or long-term care setting.
Prefer working knowledge of Personal Care Services and the 3L Policy or experience in assisted living.
Prefer working knowledge of Adult Care Home Policies and Procedures and NC DHSR requirement for the operation of an Adult Care Home.
Must have a valid NC Driver's License.
Must have or obtain Medication Aide and medication aide training within 30-days of hire.
Must be able to Lift, Push & Pull a minimum of 30 Pounds
Must be able to Bend, Stoop, Squat, and reach overhead as needed.
Must be able to read, write, understand and follow directions
Must have no substantiated findings on the NC Personnel Registry or Health Care Program
Must have a clear SBI and DMV record and maintain them during period of employment
Must have a two-step tuberculin skin test (first test prior to hire and second test within two weeks of first test)
Must be able to relate positively to residents and families and to work cooperatively with other employees, and vendors
Preferred 1 year of experience working with residents who have dementia or a dementia related diagnosis.
Preferred 2 years of supervisory/management experience in an Adult Care Home or similar setting.
Must be CPR Certified.
Salary : $25 - $30
