Opportunity Details
- Collaborates to provide care coordination activities in providing comprehensive care (e.g., other healthcare providers, local health departments, social service, nutritionist, dentist, community-based organizations, home care and patient education groups).
- Provide relevant health education and instructions, counsels patients/family responsible on the impact of illness and principles of health promotion and maintenance.
- Participates in departmental meetings and interdisciplinary community meetings.
- Able to work evenings and weekends.
Salary : $131,127 - $174,836
