Physician Assistant - Cardiology

Physician Affiliate Group of New York (PAGNY) is adding a Physician Assistant - Cardiology to the group at NYC Health Hospitals/Lincoln. Lincoln Medical Center is a designated Level I Trauma and Stroke Center and has Centers of Excellence in all medical specialties, including its award-winning Cancer and Breast Centers. Opportunity Details

• Collaborates to provide care coordination activities in providing comprehensive care (e.g., other healthcare providers, local health departments, social service, nutritionist, dentist, community-based organizations, home care and patient education groups).
• Provide relevant health education and instructions, counsels patients/family responsible on the impact of illness and principles of health promotion and maintenance.
• Participates in departmental meetings and interdisciplinary community meetings.
• Able to work evenings and weekends.
