Location: Topeka, KS and surrounding areas.
Sign-On Bonus: $5,000
Job Responsibilities:
- Conduct skilled nursing visits for patients in their homes.
- Engage in direct patient interactions to assess health conditions.
- Educate patients and families about health management, including CHF, COPD, diabetes, etc.
- Provide wound care and medication management.
- Document patient conditions and report significant changes to the physician.
- Collaborate with a team of other healthcare professionals, including Physical Therapists and Occupational Therapists.
- Current RN Nursing License in the state of practice.
- Previous home health experience preferred.
- Ability to work effectively in a fast-paced environment.
- Excellent organizational and time-management skills.
- Compassionate attitude with a focus on customer service.
- Valid driver's license and access to reliable transportation.
