Sign On Bonus: $5,000
Responsibilities- Provide direct patient care in accordance with individualized care plans
- Monitor and record patient vital signs and health status
- Administer medications and treatments as prescribed by a physician
- Educate patients and families on health management and care procedures
- Collaborate with healthcare team members to optimize patient care
- Assist with daily living activities such as bathing, dressing, and meal preparation
- Document patient care activities in accordance with company policies and healthcare regulations
- Current LPN license in the state of practice
- Minimum of one year of nursing experience, preferably in home health or community health
- Compassionate and patient-centered approach to care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic computer skills and familiarity with electronic health records
- Willingness to undergo background checks and health screenings
