Oversees Case Mix process and works closely with therapy and MDS coordinators scheduling UDA’s and assessment.Monitors and completes all required processes in accordance with the Federal Medicare and PA Medicaid Guidelines. All MDS Coordinators will operate in accordance with the practice parameters as set forth in the State Nurse Practice Act according to their licensure.Coordinates and schedules care conference meetings per regulationsReview and evaluate QM’s and QRP reports and attend meetings to discuss Participates in triple check process.Requirements for a MDS Coordinator with Phoebe Richland:The ability to deliver excellent customer service to all residents, families, and coworkers.Graduate of an approved school of nursing and currently licensed in Pennsylvania to practice as a Registered/ Licensed Nurse MDS Coordinator and case mix experience is preferredCandidates who excel are those who can thrive in a positive and respectful Customer Driven Culture while embodying the organization’s Faith in Action Mission, Vision, and Values.  Phoebe is an equal opportunity employer.
