Mission-Driven Culture – Join a team rooted in respect, integrity, and service.Responsibilities for a Hospital Liaison with Phoebe Richland:Responsible for conducting assessments for prospective residents prior to admissions; acts as a liaison to area hospital discharge planners; represents the mission and ministry of Phoebe Ministries.Experience for a Hospital Liaison with Phoebe Richland:The ability to deliver excellent customer service to all residents, families, and coworkers.Graduate of an approved School of Nursing required; current Pennsylvania License to practice as a Registered Nurse preferred or equivalent experience. Prior experience in working with geriatric patients preferred.Proficient reading, writing, grammar, and mathematics skills; demonstrated ability in dealing with people; knowledge of hospital admission and discharge procedures; knowledge of MDS form and computer helpful.Requirements for a Hospital Liaison with Phoebe Richland:The Hospital Liaison must possess or develop an affinity to Phoebe’s mission to enrich the lives of our seniors, their families, and the communities we serve. Responds to referrals by hospital discharge planners by making on-site hospital visits and managing multiple referral systems.Makes visits to assess prospective residents in their own homes, nursing homes, and assisted living/personal care facilities.Assists prospective residents and their families in completing application.Assesses prospective resident’s medical condition and needs and recommends appropriate level of care. The assessment will include diagnoses, medications, functional status, past history, and long-term plans.
