What are the responsibilities and job description for the RN - OPWDD position at Phaxis?
The Registered Nurse (RN) shall be responsible for the supervision of unlicensed direct care staff in the performance of nursing tasks and activities.
- Provide nursing care to individuals with disabilities in a community/residential setting
- Utilize the nursing process to assess, plan nursing care, implement and evaluate health outcomes of persons supported
- Assist with certification and re-certification of Approved Medication Administration Personnel (AMAP) trainings for Direct Support Professionals (DSP)
- Assist with CPR/1st Aid trainings and certification
- Obtain and interpret clinical data, follow up with medical provider's orders as necessary
- Work closely with medical coordinator to ensure timely delivery and ongoing refills of all medications
- Manage a caseload size within OPWDD parameters
- Develop individualized nursing care plans based on provider's orders, referrals and recommendations
- Initiate provider's standing orders and provide education to residential staff for of standing orders
- Collaborate and communicate with Directors, Residential Management and Staff for clinical care
- Complete initial, annual and semi- annual nursing assessments for persons supported
- Participate on incident review committee (IRC) & human rights committee (HRC), as needed
- Attend nursing trainings to remain current with evidence-based practice care as per OPWDD, NY state and company guidelines.
- Granted access to EPHI based on job title and work location
- Comply with applicable legal requirements, standards, Compliance Program: Code of Business Conduct, HIPAA, and documentation Standards
- Other duties as assigned within the nursing scope of practice
- NYS RN Certification
- Bachelor's in Nursing (BSN) Degree highly preferred
- 1 years of experience of OPWDD exp. working with individuals with intellectual and or developmental disabilities, and or behavioral health.
