Care Manager - Nurse Practitioner (REMOTE)

Location: 100% Remote
Schedule: Monday-Friday | 8:30 AM-5:00 PM (37.5 hrs/week)

We are seeking an experienced Nurse Practitioner Care Manager to join a Centralized Care Plan Team, supporting high-acuity and frail populations through coordinated, interdisciplinary care planning.
What You’ll DoLead and oversee holistic care planning in collaboration with the Interdisciplinary Team (IDT)
Review clinical documentation for quality, compliance, and accuracy
Coordinate services across settings (home care, rehab, nursing facilities, community services)
Coach and support field staff to ensure high standards of care delivery
Monitor care plans, outcomes, and utilization to ensure goals are met
Collaborate with PCPs and specialists to adjust care plans as conditions change
Ensure documentation meets federal, state, and organizational standards
Board-Certified Nurse Practitioner (NY licensed, unrestricted)
Active DEA License - required
Active Medicare & Medicaid IDs - required
Minimum 2 years of care planning experience
Background in home care, long-term care, acute care, or managed care
Experience working with frail or elderly populations strongly preferred
Strong clinical judgment, documentation, and care coordination skills
Supervisory or coaching experience a plus
Fully remote schedule
Centralized, clinical leadership role (non-field based)
