- Performing client assessments and developing individualized care plans based on agency standards.
- Managing case coordination, supervision, and implementation of nursing and supportive services.
- Responding to referrals, inquiries, and initiating appropriate in-home care actions independently.
- Ensuring compliance with agency policies, procedures, and professional standards.
- Performing additional duties as assigned by the Director of Clinical Services.
- Current RN license in Colorado.
- Minimum of two years of nursing experience in long-term care, dementia care, or acute care; home care experience preferred.
- Graduate of an accredited nursing program; Baccalaureate degree preferred.
- CPR certification required.
