Physical Therapist (Sign-On Bonus)

Role: Licensed Physical Therapist delivering in-home and senior living care to older adults, primarily through the Geriatric House Calls™ model.

Responsibilities

• Provide one-on-one physical therapy sessions in private residences and senior communities.
• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapies to enhance mobility, independence, and quality of life.
• Document patient progress accurately within electronic medical records in compliance with regulations.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety, function, and aging in place strategies.
• Incorporate telehealth and virtual care to extend access and flexibility.
  
  

Qualifications & Skills

• Valid PT license or eligibility in practice state(s).
• Degree from an accredited Physical Therapy program.
• Proficiency in basic computer use and EMR systems.
• Current CPR certification.
  
  

Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, repositioning, and mobility activities, including lifting and climbing stairs; capable of driving to patient locations and wearing PPE as needed.
