Licensed Practical Nurse
Role: Provide home health nursing visits, utilizing the nursing process to assess, plan, and deliver patient care. Act as a leader within the interdisciplinary team by communicating, delegating, and managing resources effectively. Adapt care and communication strategies to meet diverse patient needs, respecting privacy and cultural differences.
Responsibilities: Conduct patient assessments, implement teaching strategies, ensure compliance with laws and regulations, and maintain high standards of patient safety and privacy. Remain knowledgeable about current standards and promote positive outcomes and satisfaction.
Qualifications & Skills: Valid LPN license, minimum 1 year of home health experience, proficiency in electronic medical records, CPR/BLS certification, and a valid driver’s license. Strong communication, organizational, and interpersonal skills are essential. Ability to work independently and demonstrate good judgment.
Physical & Environmental Demands: Ability to perform heavy physical tasks, work in patient care environments, and handle exposure to biological and hazardous materials. Reasonable accommodations can be provided for disabilities.
