The Pepin County Human Services Department is recruiting a full-time Social Worker/Case Manager in the programs of Children’s Long-Term Support and Comprehensive Community Services. This assignment serves children and youth with medical, physical, developmental, emotional, and mental health support needs through the Children's Long-Term Support (CLTS) Program. The employee functions as the county's service coordinator/service facilitator for assigned CLTS participants and is responsible for planning, arranging, coordinating, monitoring, and documenting services in accordance with applicable Wisconsin Statutes, including chapters 46 and 49, Medicaid and HCBS waiver requirements, and county and program standards. This assignment also provides service facilitation and case management within the Comprehensive Community Services (CCS) program for individuals across the lifespan with mental health and/or substance use service needs. The employee fulfills requirements of the CCS Service Facilitator role as outlined in Wisconsin Administrative Code DHS 36 and is responsible for assessment, service planning, team coordination, documentation, monitoring, and coordination of psychosocial rehabilitation and recovery-oriented services. This position also includes on-call duties for both social service programs and mental health/AODA Crisis Intervention along with access calls.
Qualifications Include: graduation from accredited college with a Social Work degree or related filed. The candidate must be eligible for certification as a WI Social Worker.
The 2026 Wage scale for the position is dependent on experience at $27.51-$31.44 per hour. With the social work certification, the 2026 wage scale is $30.02 – $34.30 per hour. This is a full-time position scheduled Monday through Thursday 8:00 a.m. to 5:00 p.m. and Friday 8:00 a.m. – 11:30 a.m. A complete benefit package including Wisconsin Retirement, PTO available to use upon hire. This position will be scheduled on a rotating basis for on-call services evenings, weekends, and holidays with five other workers.
