Job Summary
PDS is seeking a dedicated and compassionate Social Services Case Manager to join our team. The Case Manager will provide direct support to individuals experiencing housing instability, unemployment, food insecurity, limited access to healthcare, transportation challenges, and other social service needs.
The Case Manager will help clients develop and achieve individualized goals by connecting them to housing, employment, health insurance, benefits, and community resources. Each Case Manager will maintain a caseload of approximately 16–20 clients and collaborate closely with the support team to ensure coordinated, holistic care.
Responsibilities
- Provide direct case management services to clients, supporting them in meeting individualized goals.
- Assist with housing navigation, employment access, benefit applications, and health insurance enrollment.
- Connect clients with community resources related to food, transportation, healthcare, childcare, and other essential needs.
- Develop, implement, and regularly update client service plans.
- Maintain accurate, timely documentation, case notes, and reports.
- Coordinate care with internal staff and external providers to ensure seamless service delivery.
- Monitor client progress and adjust plans as needed to support successful outcomes.
- Ensure compliance with program standards, policies, and applicable regulations.
- Develop and implement program policies, procedures, and improvements to enhance service quality
- Monitor client outcomes and adjust care plans as necessary to promote positive behavioral health and developmental progress
Experience
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field OR 3–4 years of experience in human services, social services, case management, or related direct service work.
- Strong understanding of community resources, public benefits, housing programs, and employment supports.
- Excellent communication, documentation, and organizational skills.
- Ability to build positive, professional relationships with clients, families, and community partners.
- Experience managing a caseload and working in a fast-paced, client-centered environment.
- Cultural responsiveness and the ability to work effectively with diverse populations.

Job Type: Part-time
Pay: $21.00 - $24.00 per hour
