Individual Supports Counselor - Full Time
$19.00 / hour
$20.00 / hour (after one year of benefit eligible employment)
Schedule: Monday-Thursday 3pm-10pm, Saturday 12pm8pm

Our Individual Support Program is looking for compassionate people to serve as an IS Counselor providing premier services to the individuals in our care.
What does the job entail?
You'll be supporting adults with development disabilities in Individuals' home. Individual Support Counselors implements individualized programs as specified in Individual Service Plans, provides for individuals’ health and welfare, and ensures general cleanliness and safety in individuals’ homes.
As a Individual Support Provider You Will Provide Opportunities For Individuals To Integrate Into The Community And Enjoy Outings That Interest Them. Examples Of Some Outings Are

- Providing Individual community outings
- Encourage rapport with local resources
- Instruct and assist with meal preparation
- Assistance with activities of daily living
- Instruct and physically assist with housekeeping activities
- Medical appointments
- Administer and documentation of medication and/or monitor medical health
- Assist with financial management
- Ensure home is safe
As a Individual Support Provider You Will Provide Opportunities For Individuals To Integrate Into The Community And Enjoy Outings That Interest Them. Examples Of Some Outings Are
