Part-Time | 6 Hours Per Day | Monday–Sunday
Penn Home Health Care is seeking a compassionate, dependable, and professional Licensed Practical Nurse (LPN) to provide one-on-one care for an adult participant in the home setting in Brookhaven, Pennsylvania.
This position offers a consistent schedule, supportive clinical oversight, and the opportunity to provide meaningful, person-centered care in a home environment.
Compensation
$35.00 per hour

Responsibilities
Provide skilled nursing services in the home setting
Administer medications and treatments as ordered
Monitor and assess participant condition
Observe and report changes in condition to the supervising RN and physician as appropriate
Monitor skin integrity and implement preventive measures
Assist with mobility, positioning, and safety monitoring as needed
Reinforce physician orders and care plan interventions
Communicate effectively with family members and the interdisciplinary care team
Complete accurate and timely documentation
Maintain compliance with agency policies, state regulations, and professional standards of practice
Qualifications
Current Pennsylvania LPN license in good standing
Current CPR certification
Valid driver's license and reliable transportation
Ability to work independently in a home care setting
Strong assessment, communication, and documentation skills
Home care experience preferred but not required
