What are the responsibilities and job description for the Utilization management coordinator/Social Worker position at Penn Highlands Healthcare?
AS A UTILIZATION MANAGEMENT COORDINATOR/SOCIAL WORKER, you will be responsible for preadmission, admission, continued stay, clinical quality and cost effective outcomes for a caseload of patients. Facilitates coordination of an interdisciplinary plan of care to guide patient progress toward achieving clinical, quality and cost effective outcomes. Collect specific data elements and/or clinically significant findings from the medical record to identify best practices and targeting performance improvement efforts. Completes special studies in accordance with established approved indicators and refers appropriate cases to the Quality Management Team. Performs CDMP (Clinical Documentation Management Program) to obtain complete documentation in the medical record that accurately reflects severity of illness and intensity of service leading to the most appropriate DRG/Per Case assignment. The position requires self- direction, motivation, organization, and interpretation of information relating to a wide variety of policies, procedures, and programs. This position requires extensive internal and external contacts with staff, physicians, management, and community resources. Social Worker: In collaboration with other health professionals, provides direct and indirect Social Work services to the inpatient population of the Behavioral Health Departments. Provides patient/family evaluation, history taking and discharge planning. Functions as an active participatory member of the Behavioral Health Department and maintains close communication with the Outpatient community Mental Health Service Providers.
QUALIIFICATIONS:
- Bachelor's degree (B. A.) from four-year college or university in social work or a closely related human services field and a minimum of one year related experience
- Ability to handle extreme emotional circumstances of patients required
- Understanding of interperonal and group dynamics, experience in working with agencies/professional groups
