Responsibilities

• Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary.
• Monitor and observe patients’ conditions regularly, recording vital signs and reporting any changes to healthcare providers promptly.
• Assist with medication administration following prescribed instructions while maintaining strict adherence to HIPAA regulations for patient privacy.
• Support patients with meal preparation and nutritional needs, ensuring dietary restrictions are followed accurately.
• Manage medical documentation and update electronic health record (EHR) or electronic medical record (EMR) systems efficiently to maintain accurate patient records.
• Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall health and safety.
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory-related conditions, including memory care techniques and behavioral support.
• Assist with catheter care, hospice care needs, and other complex medical procedures as trained.
• Work effectively within assisted living or long-term care settings, supporting residents’ physical and emotional needs with patience and professionalism.
• Collaborate with healthcare teams to ensure comprehensive patient care plans are followed accurately.

Experience

• Prior experience working in home health care or senior care environments is highly preferred.
• Experience with nursing home settings or long-term care facilities is advantageous.
• Familiarity with medical terminology, medical documentation practices, and EHR/EMR systems is essential.
• Demonstrated ability to work with individuals with disabilities or developmental disabilities is desirable.
• Certification in CPR and first aid is required; additional certifications such as hospice care training or dementia care are a plus.
• Proven ability to handle sensitive situations with compassion while maintaining patient confidentiality under HIPAA guidelines.
